As a courtesy to our patients, we will bill your insurance company directly for your medical visits.
We accept most major medical insurances, including but not limited to Aetna, Cigna, United Healthcare, Oxford, Amerihealth, Medicare, Horizon Blue Cross Blue Shield, Healthnet, and GHI.

We recommend that you review your insurance plan for co-pay, deductible and other benefit information prior to your office visit. Please bring your insurance card (and your referral or authorization, if applicable) to your appointment.

You may be responsible for the following charges at the time of your office visit:

• Annual insurance deductible
• Co-payments (due at time of check-in)
• Charges for services and supplies that are not covered by your insurance plan
• Any services denied by insurance
• Cosmetic services

We accept cash, checks, VISA, MasterCard and Discover for payment.

Cosmetic services are not billable to insurance. Payment for cosmetic and any other non-covered services is due in full at the time of your office visit.

Insufficient funds (bounced check) policy: patient will be billed amount due plus $30 per occurrence.

Missed appointments will be billed $50.00 unless appointment is canceled 24 hours in advance.
